Introduction: Obstetric fistula constitutes a significant public health challenge within resource-limited nations, most notably in the Democratic Republic of the Congo. This study aims to examine the epidemiological profile of patients who experienced a failure following the repair of an obstetric fistula in the city of Kindu, located in the Maniema province.
Methods: This research is based on a quantitative, analytical retrospective study utilizing the medical records of 139 women who underwent fistula intervention at the Kindu General Reference Hospital. Data collection was conducted using KoboCollect, while statistical processing was performed via STATA, employing descriptive statistics as well as bivariate analyses (OR, 95% CI, Chi-square test).
Results : The failure rate for surgical repair is 43.2 %. The primary factors associated with such failure include youth (20-24 years, p=0,000), illiteracy (p=0,000), rural residence (p=0,002), and early marriage (p=0,001). Regarding obstetrical and clinical aspects, the risk increases significantly in the absence of birth assistance (p=0,000), following prolon-ged labor (p=0,003), or following a cesarean section (p=0,043). In a logistic regression analysis, the two primary anatomical determinants that most substantially increase the risk of failure are the presence of a large fistula (OR= 13,33 ; 95 % CI : 3,84-46,24 ; p = 0,001) and a vesico-uterine location (OR= 22,29 ; 95 % CI : 7,33-67,79 ; p = 0,001).
Discussion: These findings highlight the multifactorial nature of fistula repair failure in Kindu. To reduce these failure rates and improve the prognosis for the women involved, it is imperative to facilitate access to quality obstetric care, strengthen surgical expertise, prioritize early intervention, and provide socio-economic support to patients.
